Job Summary
Utilizing the principles of the case management process, conducts assessments, plans, monitors and mobilizes resources to meet family needs. Maintains funded enrollment at assigned center.
Essential Duties & Responsibilities
(In addition to the following, other duties may be assigned)
Recruits and enrolls eligible children for Head Start/EHS/other early childhood programs, in accordance with federal, state or local regulations.
Maintains and monitors children’s attendance on a daily basis and initiates interventions for children with an ADA of 85% or less.
Completes, documents and monitors the family assessment and goal setting process for all families enrolled in the program.
Makes referrals to community agencies for social services, health, mental health, and employment and education needs of the families.
Collaborates with the Center Coordinator to organize and conduct Multi-Disciplinary Team (MDT), IEP/IFSP meetings and Child and Family Review (CFR) in order to coordinate individualized services for each child and family.
Collaborates with the Center Coordinator to document and report in-kind hours to designated agency staff.
Collaborates with the Center Coordinator to plan, organize and attend parent meetings.
Provides opportunities to actively encourage parents’ participation in the child’s education and development through parent engagement, parents as partners’ activities, volunteer in-kind hours and parent meetings.
Ensures compliance with parent involvement requirements.
Performs required data entry functions which include case notes that reflect the status of goal attainment, family situation and follow-up for referrals. Documents at least one contact per month with each assigned family.
Works with families to ensure that medicals, dental exams and treatments are complete through linkage to medical and dental providers and participation at mass screenings.
Conducts at least one home visit with all assigned families per program year.
Confers with physicians, caseworkers, other professional staff and other community partners to establish and monitor interventions on behalf of the child and family.
Implements the CDCFC Family Service Worker competencies as outlined.
Attends workshops and training programs in order to deliver and coordinate agency services (i.e., disabilities, mental health) as necessary for children and families.
May be required to travel to other centers to provide coverage based on agency needs.
Reports all cases of suspected child abuse/neglect to Franklin County Children Services according to agency procedures and the Ohio Revised Code Section 2151.421.
Required Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
Education and/or Experience
Bachelor’s Degree in social work or related degree and two (2) years of social service experience. Knowledge of principles and practices of social work, principles and techniques of interviewing and assessment, and knowledge of diverse ethnic groups, community resources, agencies and organizations.
Language Skills
Ability to read and interpret documents such as operating instructions and procedural manuals. Ability to write routine reports and correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to calculate figures and amounts and prepare reports using figures.
Reasoning Ability
The ability to use sound judgement to make well thought-out and informed decisions, the ability to apply knowledge and understanding, and use critical thinking skills to carry out instructions furnished in written, oral, or diagram form. Employees must be able to problem solve situations that involve several variables.
Technology Skills
Ability to use word processing skills, spreadsheet skills, database skills, electronic presentation skills, web navigation skills, web site design skills, e-mail management skills, computer network knowledge applicable to your department, file management & windows explorer skills, computer-related storage devices (knowledge: disks, CD’s, USB drives, zip disks, DVDs, etc.);
Physical Demands
Regular local and occasional out of town travel is required. While performing the duties of this job, the employee is frequently required to sit, stand, walk, talk and hear. The employee must be able to lift and/or move up to 35 pounds. Some travel is required.
Mental Fitness
Ability to focus, make clear decisions, use judgement, be stable and can control emotions, remain calm under adversity, ability to handle stress; be able to remain calm and professional in the face of challenging behaviors.
Work Hours
40 hours/week, generally Monday-Friday, with some evening/weekend work required. Must be able to work flexible hours between 7am and 6pm, as assigned.
Work Environment
The noise level in the work environment is usually moderate to high. The employee occasionally works in outside weather conditions.
